ａ．Ｇｅｎｅｒａｌ Ａｓｓｅｓｓｍｅｎｔ
Ｉｎｓｐｅｃｔｉｏｎ ： 健康状態把握（呼吸困難の症状の有無などを確認)、立位・歩行時の特徴を観察（姿勢、手足の動き、対称性、外傷など）、衛生状態観察(セルフケア)、身体・口腔からの異臭の有無、服装、
体重・身長の計測 ： 異常な体重増減、栄養状態
体温計測(口腔) ： ３５．８℃〜３７．３℃ （９６．４°Ｆ〜９９．１°Ｆ）
脈拍計測 ： ６０〜１００ 回数、リズム、強さ、血管の形、左右・上下差
呼吸数計測 ： １２〜２０ 回数、リズム、深さ
ｂ．Ｓｋｉｎ Ｅｎｇｌｉｓｈ
Ｉｎｓｐｅｃｔｉｏｎ ： 頭部から爪先まで、それぞれの色、色素沈着、外傷の有無などを観察。ほくろなどは直径を計測、突出しているか、最近変化があったかなどをチェック。
Ｐａｌｐａｔｉｏｎ ： それぞれの部位の、保湿度、温度、厚さ、ｔｕｒｇｏｒを触診。
ｃ．Ｈａｉｒ Ｅｎｇｌｉｓｈ
Ｉｎｓｐｅｃｔｉｏｎ ： 頭皮の表面、髪の毛を視診し、外傷の有無、頭髪の量、質、色などを観察。
Ｐａｌｐａｔｉｏｎ ： 頭皮をくまなく観て、部分的な脱毛、腫脹、外傷、色素の変化などと、毛髪の質をチェック。
ｄ．Ｎａｉｌｓ Ｅｎｇｌｉｓｈ
Ｉｎｓｐｅｃｔｉｏｎ ： 手、足の爪を視診し、爪全体の形と先端の形、色、厚さ、みぞ等の有無、清潔さを観察。
Ｐａｌｐａｔｉｏｎ ： 厚さ、みぞ等の有無、質などを触診。
Inspect the skin for appearance, color, and pigmentation.
The normal range of skin color varies from whitish pink to olive tones or to deep brown. The color and pigmentation should be uniformed and consitent over the body surface.
Abnormal findings : hyperpigmentation, cyanosis, moles, pallor, jaundice, rush, scar, lesion etc.
Palpate the skin for temperature, moisture, mobility, and turgor.
The skin sould be warm and dry without being hot, and also intact without cracking, flaking, or scaling. Picking up and slightly pinching the skin under the clavicle, the skin sould move easily when lifter and should return to place immediately when released.
Abnormal findings : dryness, flaking, cracking, poor skin turgor etc.
Inspect and Palpate the scalp and hair for surface characteristics, hair distribution, texture, quantity, and color.
The scalp should be smooth and show no evidence of flaking, scaling, redness, or open lesions. The hair sould be shiny and soft. The texture of hair should be fine or coarse.
Abnormal findings : isolated hair loss, lice etc.
Inspect and Palpate the nails for shape, contour, consistency, color, thickness, and cleanliness.
The edge of the nails may be smooth and rounded. The curvature of the nail surface should be flat and slightly curved, The nail should be smooth and have a uniform thickness. The nail base should feel firm and adhere to the nail bed.
Abnormal findings : clubbing, grooves, depressions, pitting, ridges etc.
